PROOF APPROVAL FORM

 

Every effort has been made to ensure the accompanying proofs are correct to the best of our knowledge and ability. It is, however, your final responsibility to examine them and make certain that all numbers, spellings, word usages, graphics and photos, where applicable, are correct. Our responsibility for correcting these errors are that we will make any corrections at this point free of charge, as long as they are our errors. Any alterations (changes in the copy or layout) may require us to charge you for the additional time and/or materials needed to make these alterations. If this is the case, we will inform you of the extra charges and get your approval to proceed. If these proofs are approved and printed (or processed further as necessary) and then an error is detected, we are not responsible for the costs associated with doing the work over.

According to our terms and conditions, you agree to the following:

7. Alterations Alterations represent work performed in addition to the original specifications. Such additional work shall be charged at current rates and be supported with documentation upon request.

8. Prepress Proofs Prepress proofs shall be submitted to customer for review. Corrections are to be made on these proofs and returned marked "O.K. as is" or "O.K. with corrections," or "Not O.K.: Make changes and show new proofs" and signed by customer. No further proofs will be shown unless marked "Not O.K.: Make changes and show new proofs" without prior agreement. Printer cannot be held responsible for errors under any or all of the following conditions: If the work is printed per customer's O.K.; if changes are communicated verbally; if the customer has failed to return proofs with indication of changes; or if customer has instructed printer to proceed without submission of proofs.
